Necrobiotic xanthogranuloma (NXG) can present as a yellowish plaque within linear morphea. Surgical excision of this rare condition led to no recurrence over 12 months.

Area of Science:

  • Dermatology
  • Pathology
  • Granulomatous Diseases

Background:

  • Linear morphea is a rare autoimmune connective tissue disease.
  • Necrobiotic xanthogranuloma (NXG) is an uncommon granulomatous condition often associated with paraproteinemia.

Observation:

  • A 32-year-old woman presented with a yellowish atrophic plaque within existing linear morphea on her left arm.
  • Photographic review confirmed the lesion was present for at least 3 years and had enlarged.
  • Biopsy confirmed the diagnosis of Necrobiotic xanthogranuloma (NXG).

Findings:

  • The patient's NXG was not associated with detectable paraproteinemia.
  • Surgical excision was performed for the NXG lesion.
  • No recurrence of NXG was observed during a 12-month follow-up period.

Related Experiment Videos

Implications:

  • This case highlights a rare presentation of NXG within a morphea lesion.
  • It suggests NXG can occur in the absence of paraproteinemia.
  • Surgical management appears effective for localized NXG in this context.
