    Summary

    Hypercalcaemia in chronic lymphatic leukaemia (CLL) can be refractory to standard treatments. Osteoclast-stimulating factors may drive this severe complication, as evidenced by bone resorption.

    • Hypercalcaemia is a potential complication of various malignancies.

    Observation:

    • A patient with CLL developed hypercalcaemia unresponsive to conventional therapies.
    • Bone biopsy revealed significant osteoclastic resorption.

    Findings:

    • Review of 7 literature cases links hypercalcaemia with CLL.
    • Evidence suggests osteoclastic activity is a key feature.

    Implications:

    • Osteoclast-stimulating factors are implicated in CLL-associated hypercalcaemia.
    • Understanding these mechanisms may lead to novel therapeutic strategies.
