Summary

Localized facial edema in a patient with chronic urticaria and angioedema symptoms was linked to thyroid autoimmunity. Treatment with thyroxin (thyroid hormone) effectively resolved the edema, demonstrating a connection between thyroid antibodies and localized swelling.

Background:

Observation:

  • A 42-year-old woman presented with a two-month history of right hemifacial edema.
  • Standard allergy testing (skin and epicutaneous) was negative.
  • Thyroglobulin antibodies were elevated (492 IU/mL), while microsomal antibodies were negative.

Findings:

  • Facial edema resolved with two months of thyroxin treatment.
  • Relapse of edema occurred as thyroglobulin antibody levels increased despite initial decrease.
  • Re-initiation of thyroxin treatment led to symptom resolution again.
