Lung decortication significantly improves lung perfusion and spirometry in chronic pleural empyema patients. However, affected lung function remains impaired post-surgery.

Background:

  • Chronic pleural empyema (CPE) significantly impairs lung function.
  • Surgical intervention, such as decortication, is often necessary to treat CPE.

Purpose of the Study:

  • To evaluate lung function changes after lung decortication in CPE patients.
  • To assess the impact of decortication on lung perfusion and spirometry.

Main Methods:

  • Prospective evaluation of 26 CPE patients.
  • Pre- and post-operative assessments included lung perfusion scintigraphy, blood gas analysis, and spirometry.
  • Follow-up was conducted 35 weeks (+/-17) after decortication.

Main Results:

  • Preoperative lung perfusion was significantly reduced on the affected side (e.g., 24.5% for right CPE).

  • Postoperative perfusion improved substantially (e.g., to 45.2% for right CPE).
  • Vital capacity (VC) improved from 62.3% to 79.8%, and FEV1 from 50% to 69.2% post-surgery. Blood gas analysis also showed improvement.
  • Conclusions:

    • Lung decortication leads to significant improvements in lung perfusion and spirometry in CPE patients.
    • Despite improvements, residual impairment of affected lung function persists after decortication.
    • Patient age, gender, disease side, bacteriology, or duration did not influence lung function outcomes.
