Abstract:
Daily intake and output (I&O) and weight documentation is frequently incomplete or omitted completely, even when a physician's order is in place. This brief case study illustrates the importance of maintaining these records to avoid fluid balance disorders and negative patient outcomes, as well as support patient well-being.
