Background:

  • Actinic prurigo is a rare, severe photosensitivity disorder.
  • Human Leukocyte Antigen (HLA) associations, particularly HLA-DR4, are noted in actinic prurigo.

Observation:

  • A 35-year-old male presented at age 8 with recurrent, pruritic, papulovesicular lesions triggered by light exposure.
  • Diagnosis of actinic prurigo was confirmed by HLA-DR4, specifically the DRB1*0407 subtype.

Findings:

  • Thalidomide treatment initiated at age 11 led to rapid lesion resolution.
  • Long-term maintenance therapy (50 mg/day for 23 years, cumulative dose >400g) effectively controlled photosensitivity.
  • No adverse effects, including neuropathy, were observed during the extended treatment period.

Implications:

  • This case demonstrates the potential for safe and effective long-term thalidomide use in managing severe actinic prurigo.
  • Highlights the importance of HLA genotyping in diagnosing and managing this condition.
  • Suggests thalidomide as a viable therapeutic option for recalcitrant cases of actinic prurigo.
