Area of Science:

  • Endocrinology
  • Surgical Pathology
  • Diagnostic Imaging

Background:

  • A 63-year-old male presented with dyspnea and a neck mass.
  • Imaging revealed a cyst extending from the thyroid to the superior mediastinum, causing tracheal deviation.

Observation:

  • Fine-needle aspiration initially resolved the cyst, but fluid reaccumulated.
  • Thyroid and parathyroid function tests, along with blood chemistry, were normal.

Findings:

  • Surgical excision and analysis confirmed a nonfunctioning parathyroid cyst.
  • High intact parathyroid hormone levels in cyst fluid supported the diagnosis.

Implications:

  • This case highlights the importance of considering parathyroid cysts in the differential diagnosis of neck masses.
