Summary
A child developed severe hypertension after a kidney transplant biopsy. Arteriovenous fistula (AVF) was found and successfully treated with embolization.

Background:
- Percutaneous kidney transplant biopsy is a common diagnostic procedure.

Observation:
- A 6-year-old boy presented with severe, medication-resistant hypertension one week post-renal transplant biopsy.
- The hypertension emerged shortly after the percutaneous biopsy procedure.
Findings:
- Renal angiography identified an arteriovenous fistula (AVF) at the biopsy site in the lower pole of the transplant kidney.
- The arteriovenous fistula was the cause of the refractory hypertension.
Implications:
- Arteriovenous fistulas are a potential complication of percutaneous renal procedures.
- Embolization is an effective treatment for post-biopsy arteriovenous fistulas, resolving hypertension.
