Summary

The Cordis Keeper filter showed good tolerance in pigs, but long-term use may increase the risk of caval wall penetration due to filter span expansion in smaller inferior vena cava (IVC) vessels.

Area of Science:

  • Vascular surgery
  • Medical device evaluation
  • Histopathology

Background:

  • Inferior vena cava (IVC) filters are used to prevent pulmonary embolism.
  • Long-term interaction between IVC filters and the vessel wall requires further investigation.

Purpose of the Study:

  • To evaluate the interaction between the Cordis Keeper vena caval filter and the vessel wall in a porcine model.

Main Methods:

  • Implantation of the Cordis Keeper filter in five pigs.
  • Collection of radiologic data on IVC diameter, filter patency, leg span, and stability.
  • Histopathologic analysis of the IVC wall at 2 or 6 months post-implantation.

Main Results:

  • All filters remained patent without migration.

Related Experiment Videos

  • At 6 months, two filter legs penetrated the vessel wall and adhered to the liver.
  • A gradual increase in filter span correlated with increased risk of caval wall penetration, particularly in smaller IVCs (average diameter 16 mm).
  • Conclusions:

    • The Cordis Keeper filter was well tolerated in the short term.
    • Long-term use may pose a risk of caval wall penetration.
