Summary

Diclofenac alone or with alfentanil patient-controlled analgesia (PCA) provided similar pain relief during shock wave lithotripsy. However, patients reported higher satisfaction when using PCA, though most preferred diclofenac alone if treated twice.

Area of Science:

  • Urology
  • Pain Management
  • Nephrology

Background:

  • Shock wave lithotripsy (SWL) is a common procedure for kidney stone removal.
  • Effective analgesia is crucial for patient comfort and procedural success during SWL.

Purpose of the Study:

  • To compare the effectiveness and patient preference of diclofenac alone versus a combination of diclofenac and alfentanil patient-controlled analgesia (PCA) during SWL.

Main Methods:

  • A randomized crossover study involving 64 patients undergoing SWL.
  • Patients received either diclofenac alone or diclofenac with alfentanil PCA.
  • Pain and satisfaction were assessed using a visual analog scale.

Main Results:

  • No significant difference in mean pain scores between the two analgesic groups (p = 0.34).

Related Experiment Videos

  • Patients receiving alfentanil PCA reported significantly higher satisfaction (p = 0.04).
  • Despite higher satisfaction with PCA, 58% of patients treated twice preferred diclofenac alone.
  • Conclusions:

    • Diclofenac alone and diclofenac with alfentanil PCA offer comparable pain management during SWL.
    • Patient-controlled analgesia enhances satisfaction, but preference may vary.
    • Further research may explore optimal analgesia strategies for SWL.
