Thyroid carcinoma. Correlation between clinical and pathological findings.

E Pimentel, M Sorgi, M Paolillo

    International Surgery
    |October 1, 1975
    PubMed
    Summary

Thyroid carcinoma presents variably, with diffuse enlargement in 20% of cases. Multiple carcinoma foci are common, especially after extensive thyroidectomy, and anaplastic types affect all ages.

    Area of Science:

    • Oncology
    • Endocrinology
    • Pathology

    Background:

    • Thyroid carcinoma is a significant endocrine malignancy.
    • Understanding its varied presentations and characteristics is crucial for diagnosis and treatment.
    • Previous studies highlight the importance of clinicopathological features in thyroid cancer management.

    Purpose of the Study:

    • To analyze the diverse clinical and pathological features of thyroid carcinoma.
    • To investigate the prevalence of diffuse enlargement, non-cold zone localization, multifocality, and anaplastic subtypes.
    • To correlate surgical extent with the incidence of multifocal disease.

    Main Methods:

    • Retrospective analysis of 146 thyroid carcinoma cases.
    • Review of clinical data including gland size, nodularity, and tumor location.
    • Assessment of histopathological findings, focusing on multifocality and subtype.
    • Correlation of surgical procedures (total, subtotal, partial thyroidectomy) with tumor characteristics.

    Main Results:

    • 20% of cases exhibited diffuse thyroid gland enlargement without discernible nodules.
    • Thyroid carcinoma was identified in non-"cold" zones in 35% of evaluated cases.
    • Multifocal carcinoma was observed in 67% of patients undergoing total/subtotal thyroidectomy versus 33% with partial thyroidectomy.
    • Anaplastic carcinomas constituted 13% of cases, occurring across all age groups, with 29% in patients under 30.

    Conclusions:

    • Thyroid carcinoma demonstrates significant heterogeneity in presentation, including diffuse enlargement and extranodal involvement.
    • The extent of thyroidectomy is associated with the detection rate of multifocal thyroid carcinoma.
    • Anaplastic thyroid carcinoma can occur in younger individuals, necessitating consideration in all age demographics.
