Area of Science:

  • Hepatobiliary Surgery
  • Gastroenterology
  • Surgical Oncology

Background:

  • Malignancy within choledochal cysts is an uncommon but serious complication.
  • This study details clinical presentation, management strategies, and outcomes for this rare condition.

Purpose of the Study:

  • To analyze the clinical features, treatment approaches, and survival rates of patients with malignancy in choledochal cysts.
  • To highlight the importance of suspecting malignancy in specific patient presentations.

Main Methods:

  • Retrospective review of 80 adult patients with choledochal cysts treated between 1979 and 1995.
  • Focus on the eight patients diagnosed with concurrent malignancy within the cyst.

Main Results:

  • Malignancy was found in 10% of the adult choledochal cyst cohort.
  • Common presentations included biliary tract infection, gastric outlet obstruction, and right upper quadrant pain with weight loss.
  • Surgical interventions varied, and operative mortality was observed; survival ranged from 4 to 9 months.

Conclusions:

  • Malignancy in choledochal cysts requires high clinical suspicion, especially with symptoms like cholangitis, weight loss, and anemia.
  • The prognosis for malignancy in choledochal cysts is generally poor.
  • Early detection and appropriate management are crucial for improving patient outcomes.
