Summary

Magnetic resonance (MR) imaging features can indicate sciatica prognosis in leg pain patients. However, findings like annular rupture and nerve root compression are not strong enough for routine early MR use.

Background:

  • Sciatica from disc herniation typically resolves but recovery is unpredictable.
  • Some patients require extensive conservative therapy or surgery for symptom relief.

Purpose of the Study:

  • To determine if magnetic resonance (MR) imaging can predict sciatica outcomes.
  • To identify patients unlikely to respond to conservative management using early MR imaging.

Main Methods:

  • A transversal diagnostic study included 274 primary care patients with leg pain undergoing early MR imaging.
  • One hundred thirty-three sciatica patients were followed for 3 months, blinded to MR findings.

Main Results:

  • Favorable prognosis at 12 weeks was associated with annular rupture (P=0.02) and nerve root compression (P=0.03) on MR imaging.
  • Poor prognosis was indicated by disc herniation in the foramen (P=0.004).

Conclusions:

  • Early MR imaging features correlate with sciatica prognosis.
  • The associations are insufficient to warrant routine early MR imaging for predicting sciatica prognosis.
