Vena cava umbrella filter: experimental, hemodynamic and embolization studies.

P T Harjola, K Höckerstedt, T M Scheinin

    Summary

The Mobin-Uddin umbrella filter, used in the inferior vena cava (IVC), quickly caused thrombus formation and occlusion. This study shows the filter led to significant IVC blockage in dogs within one week.

    Area of Science:

    • Vascular Surgery
    • Medical Device Engineering
    • Thrombosis Research

    Background:

    • Inferior vena cava (IVC) filters are used to prevent pulmonary embolism.
    • The Mobin-Uddin umbrella filter is a specific type of IVC filter.
    • Understanding filter performance and complications is crucial for patient safety.

    Purpose of the Study:

    • To evaluate the hemodynamic effects and embolization potential of the Mobin-Uddin umbrella filter.
    • To assess the thrombogenicity and long-term patency of the IVC after filter placement.

    Main Methods:

    • Placement of the Mobin-Uddin umbrella filter in mongrel dogs.
    • Hemodynamic measurements including blood flow and pressure.
    • Cineangiography and radiopaque emboli to study embolization.

    Related Experiment Videos

  • Autopsy to examine thrombus formation and IVC patency.
  • Main Results:

    • Initially, hemodynamics were unaffected. Flow decreased significantly by 70 minutes.
    • Emboli passed through the filter initially, but thrombus formation began within one hour.
    • By two hours, filter perforations were occluded by thrombus. Complete IVC occlusion occurred within one week.

    Conclusions:

    • The Mobin-Uddin umbrella filter demonstrated significant thrombogenicity.
    • Rapid thrombus formation led to occlusion of the filter and the infrarenal IVC.
    • These findings raise concerns about the long-term patency and safety of this filter design.
