    This study presents a surgical technique to correct retrograde ejaculation after bladder neck surgery. The procedure successfully restored normal ejaculation in two patients, with one achieving fatherhood.

    Background:

    • Retrograde ejaculation is a known complication following Y-V plasty of the bladder neck.
    • This condition significantly impacts male fertility and sexual satisfaction.

    Purpose of the Study:

    • To describe a surgical method for reconstructing the internal vesical sphincter.
    • To correct retrograde ejaculation in patients who have undergone Y-V plasty.

    Main Methods:

    • A transvesical approach was utilized for the surgical reconstruction.
    • The internal vesical sphincter was reconstructed to a standard diameter (No. 16 Foley catheter).
    • Excision of scar tissue and mucosa was performed, followed by approximation of the bladder neck muscle.

    Main Results:

    • Both patients experienced a successful correction of retrograde ejaculation.
    • One patient achieved natural conception and fathered a child post-surgery.

    Conclusions:

    • Transvesical reconstruction of the internal vesical sphincter is an effective treatment for retrograde ejaculation post-Y-V plasty.
    • This surgical intervention can restore normal antegrade ejaculation and fertility.
