    Summary

    Spinal tuberculosis in the cervicothoracic region can cause upper extremity radiculopathy, mimicking other conditions. Early diagnosis and combined surgical and chemotherapy treatment resolved symptoms in this rare case.

    Background:

    • Spinal tuberculosis (TB) is a significant cause of morbidity, particularly in developing countries.
    • Cervicothoracic junctional area involvement is less common and can present with atypical symptoms.
    • Radiculopathy, especially brachial plexus involvement, is an unusual manifestation of spinal TB.

    Observation:

    • A case is presented of a patient with cervicothoracic spinal tuberculosis.
    • The patient experienced severe radicular pain and muscle weakness in the hand, with a claw deformity, delaying diagnosis.
    • Magnetic resonance imaging (MRI) revealed advanced bone destruction and instability at the cervicothoracic junction.

    Findings:

    • The patient underwent radical anterior debridement and fusion, followed by chemotherapy.
    • Surgical and medical management led to the resolution of upper extremity symptoms.
    • Brachial plexus radiculopathy secondary to spinal tuberculosis has not been previously reported in the literature.
    • The absence of myelopathic signs despite significant spinal instability and thecal compression was noteworthy.

    Implications:

    • This case highlights the importance of considering spinal tuberculosis in the differential diagnosis of upper extremity radiculopathy, even without typical neurological deficits.
    • Aggressive surgical debridement, fusion, and appropriate chemotherapy are effective in managing this condition.
    • Further research into the specific mechanisms and clinical presentations of spinal TB affecting the brachial plexus is warranted.
