Cutaneous polyarteritis nodosa (PAN) presents with nodules and livedo reticularis, often favoring the lower limbs. While typically having a good prognosis, systemic evaluation is crucial due to potential arteritis in other organs.

Area of Science:

  • Dermatology
  • Rheumatology
  • Pathology

Background:

  • Classic polyarteritis nodosa (PAN) is a vasculitis affecting small- and medium-sized arteries.
  • A cutaneous variant of PAN, with a more favorable prognosis and no visceral involvement, was described in 1931.
  • This study reviews four cases of cutaneous PAN diagnosed between 1987 and 1998.

Observation:

  • The study group included three women and one child, aged 11 to 70 years.
  • Nodules were the most frequent cutaneous lesions, primarily on the lower limbs.
  • The erythrocyte sedimentation rate was the sole consistently altered laboratory parameter.

Findings:

  • Histological analysis revealed segmentary leucocytoclastic vasculitis in the deep dermis and/or hypodermis.
  • Direct immunofluorescence was positive in only one patient.

Related Experiment Videos

  • No evidence of visceral involvement was detected in any of the four patients.
  • Implications:

    • There is diagnostic ambiguity surrounding cutaneous PAN.
    • Clinical signs like nodules and livedo reticularis suggest a good prognosis.
    • Despite cutaneous presentation, thorough evaluation for systemic involvement is recommended due to the 'polyarteritis' nature of the condition.
