Background:

  • Spinal cord infarction is a rare but devastating neurological event.
  • Diagnosis can be challenging, especially when initial imaging is equivocal.

Observation:

  • A 65-year-old male presented with symptoms suggestive of spinal cord infarction.
  • Magnetic resonance imaging (MRI) of the spinal cord showed no definitive signs of infarction.
  • However, T2-weighted MRI revealed bone marrow hyperintensities in vertebrae Th9 and Th10, indicating vertebral body infarction.

Findings:

  • Vertebral body infarction, visualized as bone marrow hyperintensities on MRI, can be a critical diagnostic clue.
  • This finding was the only confirmatory sign in a case of presumed spontaneous spinal cord infarction.
