Summary

Alternating hemiplegia of childhood is a rare neurological disorder causing temporary paralysis. Flunarizine treatment significantly reduced the frequency, duration, and severity of hemiplegic attacks in a young patient.

Area of Science:

  • Pediatric Neurology
  • Neuroscience

Background:

  • Alternating hemiplegia of childhood (AHC) is a rare neurological disorder affecting infants.
  • Characterized by transient hemiplegic episodes that shift sides, often starting before 18 months.
  • Early symptoms like nystagmus and dystonic attacks are frequently misdiagnosed as epilepsy.

Purpose of the Study:

  • To review the current understanding of alternating hemiplegia of childhood.
  • To present a case study on the diagnostic and therapeutic aspects of AHC.
  • To evaluate the efficacy of flunarizine in managing AHC symptoms.

Main Methods:

  • Clinical diagnosis based on characteristic hemiplegic attacks.
  • Exclusion of other neurological disorders through standard investigations (EEG, MRI, etc.).

Related Experiment Videos

  • Cerebral SPECT imaging during an interictal period.
  • Main Results:

    • Standard investigations, including EEG and MRI, were normal.
    • Cerebral SPECT revealed asymmetry with hypoperfusion in the left frontal/temporal regions and hyperperfusion in the occipital area.
    • Flunarizine treatment led to a significant reduction in attack frequency, duration, and severity.

    Conclusions:

    • AHC diagnosis relies on clinical presentation and exclusion of other conditions.
    • Cerebral SPECT may show specific perfusion abnormalities in AHC patients.
    • Flunarizine is an effective symptomatic treatment for alternating hemiplegia of childhood.
