Incidental appendectomy during radical cystectomy offers no benefit. This study found no appendicitis complications in patients who did not have an incidental appendectomy, suggesting it is unnecessary.
Area of Science:
- Urology
- Surgical Oncology
Background:
- Incidental appendectomy during radical cystectomy aims to prevent future appendicitis.
- The necessity of this procedure is debated due to potential added morbidity.
Purpose of the Study:
- To evaluate the clinical rationale and outcomes of incidental appendectomy performed concurrently with radical cystectomy.
- To determine if incidental appendectomy during radical cystectomy provides any advantage in preventing future appendicitis-related complications.
Main Methods:
- A retrospective review of 105 patients undergoing radical cystectomy between 1986 and 1993.
- Comparison of outcomes between 40 patients who had incidental appendectomy and 65 patients who did not.
Main Results:
- Median follow-up exceeded 10 years for the appendectomy group and 7 years for the non-appendectomy group.
- No appendicitis-related complications were observed in either group during the follow-up period.
Conclusions:
- The data suggest that incidental appendectomy during radical cystectomy does not offer a significant advantage.
- Routine incidental appendectomy in this context may not be warranted based on current evidence.
