We describe a new cluster headache variant linked to hemiparesis. This suggests a potential channelopathy, similar to hemiplegic migraine, possibly with autosomal dominant inheritance in some cases.

Background:

Cluster headache is a primary debilitating headache disorder.
Hemiplegic migraine, a rare subtype, is characterized by hemiparesis during attacks and is linked to ion channel gene mutations.
The pathophysiology of typical cluster headache remains incompletely understood.

Observation:

Four patients presented with a novel cluster headache variant.
This variant was clinically characterized by the occurrence of hemiparesis during cluster headache attacks.
One patient exhibited a family history consistent with autosomal dominant inheritance.
