Headache·2025

We describe a new cluster headache variant linked to hemiparesis. This suggests a potential channelopathy, similar to hemiplegic migraine, possibly with autosomal dominant inheritance in some cases.

Area of Science:

  • Neurology
  • Neuroscience
  • Genetics

Background:

  • Cluster headache is a primary debilitating headache disorder.
  • Hemiplegic migraine, a rare subtype, is characterized by hemiparesis during attacks and is linked to ion channel gene mutations.
  • The pathophysiology of typical cluster headache remains incompletely understood.

Observation:

  • Four patients presented with a novel cluster headache variant.
  • This variant was clinically characterized by the occurrence of hemiparesis during cluster headache attacks.
  • One patient exhibited a family history consistent with autosomal dominant inheritance.

Findings:

  • The observed hemiparesis in cluster headache attacks suggests a potential shared mechanism with hemiplegic migraine.
  • This clinical presentation supports the hypothesis that this cluster headache variant may represent a channelopathy.

Related Experiment Videos

  • Autosomal dominant inheritance patterns in a subset of patients further implicate genetic factors, possibly ion channel dysfunction.
  • Implications:

    • This finding expands the clinical spectrum of cluster headache.
    • It suggests that ion channel dysfunction may play a role in certain cluster headache subtypes.
    • Further research into the genetic and molecular basis of this hemiplegic cluster headache variant is warranted to explore therapeutic strategies targeting ion channels.
