    Background:

    • Mediastinoscopy is a key diagnostic tool for staging lung cancer and evaluating mediastinal masses.
    • Repeat mediastinoscopy is uncommon, with limited data on its indications and safety.

    Observation:

    • Out of 1,188 mediastinoscopies, six (0.5%) required re-exploration.
    • Adhesions between the innominate artery and trachea were not always extensive, and alternative routes were feasible.

    Findings:

    • Lenient indications for repeat mediastinoscopy are supported by the low complication rate observed.
    • Earlier mediastinoscopy does not preclude a subsequent procedure.

    Implications:

    • This study suggests that repeat mediastinoscopy can be considered more liberally.
    • Careful surgical technique is advised to mitigate potential vascular complications during re-exploration.
