Summary

Stent-graft implantation effectively treated a large thrombus in the common iliac artery, preventing dangerous distal embolization. This procedure offers a viable option for similar vascular lesions.

Background:

  • Large thrombus-containing lesions in major arteries pose a risk of distal embolization.
  • Iliac artery occlusions can cause severe limb claudication.

Observation:

  • A 67-year-old male presented with disabling left leg claudication due to a common iliac artery thrombus.
  • Angiography revealed a mobile, lobular mass occluding the artery, suggestive of an atherothrombotic lesion.

Findings:

  • Percutaneous stent-graft implantation successfully excluded the thrombotic lesion.
  • The stent-graft entrapped the thrombus, confirmed by a central balloon waist.
  • Completion angiography demonstrated no residual stenosis or distal embolization.

  • Stent-graft implantation is a feasible treatment for localized, thrombus-containing arterial lesions.
  • This technique can prevent distal embolization in large vessel occlusions.
  • Successful treatment led to asymptomatic status and maintained patency at 6-month follow-up.
