Background:

  • Ovarian calcification and ossification are rare phenomena.
  • Previous reports have not described whole-ovary involvement mimicking stone formation.

Observation:

  • A 42-year-old woman presented with severe abdominal pain and a palpable adnexal mass.
  • Imaging revealed a 4x5 cm calcified mass in the right adnexa.
  • Surgical exploration showed an enlarged ovary with extensive calcification and ossification, resembling stone.

Findings:

  • Pathologic examination confirmed a benign chocolate cyst with extensive calcification and ossification.
  • The unique presentation mimicked a stone-like ovary.
  • Complete surgical excision led to complete resolution of pelvic pain.
