Summary

Lupus anticoagulant (LA) can be found in young patients experiencing epileptic seizures. This association exists even without systemic lupus erythematosus (SLE) or evidence of cerebral infarcts.

Background:

  • Lupus anticoagulant (LA) is linked to ischemic stroke in systemic lupus erythematosus (SLE) patients.
  • LA has been observed in elderly epilepsy patients, potentially due to subclinical infarcts.

Observation:

  • Two young patients (<35 years) presented with epileptic seizures and tested positive for LA.
  • Neither patient had SLE or cerebral infarcts that could explain their seizures.

Findings:

  • A 28-year-old woman with new-onset seizures and aortic insufficiency was LA positive; MRI revealed no ischemic event.
  • A 33-year-old man with intractable epilepsy was LA positive; clinical evaluation and MRI showed no antecedent ischemic event.

Implications:

  • LA may be associated with epileptic seizures in young individuals without SLE or cerebral infarcts.
  • This suggests a potential link between LA and epilepsy in younger demographics, independent of typical risk factors.
