Summary

A rare case of ocular scrofuloderma in a child presented with proptosis due to a tubercular orbital abscess. Anti-tubercular treatment successfully resolved the abscess and associated lesions without surgery.

Background:

  • Ocular scrofuloderma, a manifestation of tuberculosis (TB) involving the eye and surrounding structures, is uncommon.
  • Extraconal orbital abscesses are rare, and their association with lacrimal gland involvement and ocular scrofuloderma has not been previously documented.

Observation:

  • A 7-year-old boy presented with facial nodulo-ulcerative lesions, discharging sinuses, and proptosis.
  • A Computerized Tomography (CT) scan revealed an extraconal, intraorbital abscess involving the lacrimal gland and eroding the temporal bone.

Findings:

  • The patient received anti-tubercular treatment, leading to significant improvement in both dermatological and ophthalmological symptoms.
  • Orbital ultrasound demonstrated a progressive decrease in abscess size, ultimately resulting in its complete resolution, avoiding surgical intervention.

Implications:

  • This case highlights the importance of considering tuberculosis in the differential diagnosis of orbital abscesses, especially in endemic areas.
  • Non-surgical management with anti-tubercular therapy can be effective in resolving complex orbital tubercular abscesses with lacrimal gland involvement.
