Summary

Transperitoneal laparoscopic retropubic urethropexy using sutures showed better long-term objective success than mesh techniques for genuine stress incontinence. This surgical approach offers improved outcomes for patients.

Area of Science:

  • Urology
  • Minimally Invasive Surgery
  • Female Pelvic Medicine

Background:

  • Genuine stress urinary incontinence affects many women.
  • Surgical interventions aim to restore urethral support.
  • Transperitoneal laparoscopic urethropexy is a surgical option.

Purpose of the Study:

  • To compare two transperitoneal laparoscopic urethropexy techniques.
  • Evaluate suture versus mesh fixation for stress incontinence.

Main Methods:

  • Prospective randomized trial of 60 women with genuine stress incontinence.
  • Group A: Laparoscopic retropubic urethropexy with non-absorbable sutures.
  • Group B: Laparoscopic retropubic urethropexy with prolene mesh and tackers/staplers.
  • Evaluations included subjective (patient-reported symptoms, visual analog scale) and objective (clinical, urodynamic studies) measures.

Related Experiment Videos

  • Follow-up at 3, 6, and 12 months.
  • Main Results:

    • No significant differences in intra-operative or postoperative complications.
    • Subjective failure rates were similar between groups at all follow-up points.
    • Objective failure rates were similar at 3 and 6 months.
    • At 12 months, objective failure rates were significantly lower in the suture group (Group A) compared to the mesh group (Group B).

    Conclusions:

    • Transperitoneal laparoscopic retropubic urethropexy using non-absorbable sutures is more effective than the mesh technique for genuine stress incontinence.
    • Suture fixation demonstrates superior long-term objective efficacy.
