Summary

Multiple system atrophy (MSA), a neurodegenerative disorder, can present with psychotic symptoms like delusional parasitosis, challenging exclusion criteria. Effective low-dose olanzapine treatment showed promise for managing these symptoms in a patient case.

Area of Science:

  • Neurology
  • Psychiatry

Background:

  • Multiple system atrophy (MSA) is a progressive neurodegenerative disease with complex etiology.
  • Clinical diagnosis involves extrapyramidal, cerebellar, pyramidal, and autonomic dysfunction.
  • Hallucinations unrelated to medication are typically an exclusion criterion for MSA diagnosis.

Observation:

  • A case study details a patient presenting with delusional parasitosis as an initial symptom of MSA.
  • This challenges the established diagnostic exclusion criterion regarding hallucinations.

Findings:

  • The simultaneous occurrence of MSA and psychotic disorders, such as delusional parasitosis, is possible.
  • Delusional parasitosis is an unspecific syndrome with multifactorial causes.
  • Low-dose olanzapine demonstrated efficacy in achieving partial remission of delusional symptoms over an extended period.

Implications:

  • Re-evaluation of diagnostic exclusion criteria for MSA may be warranted.
  • Psychotic symptoms should be considered in the differential diagnosis of MSA.
  • Pharmacological management, including neuroleptics like olanzapine, can be beneficial for managing psychosis in MSA patients.
