A boost of CD34(+) cells can improve poor graft function after allogeneic bone marrow transplant (allo-BMT) when patients have full donor chimerism. This approach offers an alternative for patients without graft rejection.

Background:

  • Poor graft function affects approximately 9% of patients undergoing allogeneic bone marrow transplantation (allo-BMT).
  • This complication can lead to severe, life-threatening issues without evident graft rejection.
  • Existing treatments like growth factors, transfusions, and donor lymphocyte infusions may not resolve the issue.

Observation:

  • Three patients with acute leukemia or myelodysplastic syndrome undergoing allo-BMT experienced prolonged cytopenias.
  • Standard supportive care and donor leukocyte infusions failed to correct the poor graft function.
  • A CD34(+) cell boost from HLA-identical siblings was administered to these patients.

Findings:

  • Patients with full donor chimerism at the time of CD34(+) cell infusion showed rapid improvement in peripheral blood counts.

Related Experiment Videos

  • The patient with mixed chimerism did not exhibit a positive response to the CD34(+) cell boost.
  • No patients experienced worsening of graft-versus-host disease (GvHD) after the CD34(+) cell infusion.
  • Implications:

    • Positively immunoselected allogeneic CD34(+) cells represent a promising therapeutic option for poor graft function post-allo-BMT.
    • This strategy may be particularly effective in cases without signs of graft rejection.
    • Further research is warranted to optimize the use of CD34(+) cell boosts in managing allo-BMT complications.
