Background:

  • Intramuscular haemangiomas are benign vascular tumors that can occur in various locations.
  • Orbital involvement of intramuscular haemangiomas, particularly in extraocular muscles, is exceptionally rare.

Observation:

  • A 21-year-old female presented with progressive proptosis due to a retrobulbar lesion.
  • The lesion was painless, non-compressible, and unresponsive to three prior surgical attempts over nine years.
  • Magnetic resonance imaging (MRI) suggested an angiomatous origin for the slowly enlarging tumor.

Findings:

  • Histopathological examination confirmed a mixed-type intramuscular haemangioma involving four extraocular muscles.
  • The tumor necessitated enucleation of the eye and excision of affected muscles for radical treatment.
