Summary

Interventional EUS can create a pancreaticogastrostomy for chronic pancreatitis pain when standard endoscopic methods fail. This technique offers a promising alternative for pancreatic drainage and pain relief.

Area of Science:

  • Gastroenterology
  • Interventional Endoscopy
  • EUS (Endoscopic Ultrasound)

Background:

  • Chronic pancreatitis pain is often linked to pancreatic ductal hypertension.
  • Endoscopic drainage of the main pancreatic duct offers pain relief.
  • Conventional endoscopic drainage is not feasible when transpapillary access is unobtainable.

Observation:

  • Four patients with chronic pancreatitis pain and a dilated main pancreatic duct underwent EUS-guided pancreaticogastrostomy.
  • The procedure involved accessing the dilated duct and creating a pancreaticogastrostomy, stented for patency.
  • No major complications were observed during the EUS-guided pancreaticogastrostomy procedures.

Findings:

  • Three out of four patients experienced satisfactory pain relief.

Related Experiment Videos

  • Median follow-up was one year, demonstrating sustained pain management.
  • EUS-guided pancreaticogastrostomy provides an alternative drainage method.
  • Implications:

    • EUS-guided pancreaticogastrostomy is a potentially valuable technique for pancreatic drainage.
    • This method offers pain relief for chronic pancreatitis patients.
    • It is particularly useful when conventional endoscopic access to the pancreatic duct is not possible.
