A carcinosarcoma/sarcomatoid carcinoma arising in a urinary bladder diverticulum

Observation:

• A 65-year-old male presented with an invasive polypoid carcinosarcoma/sarcomatoid carcinoma originating within a urinary bladder diverticulum.
• Histological examination revealed a complex tumor with high-grade urothelial carcinoma, malignant glandular structures, and spindle cell components.
• The tumor demonstrated full-thickness invasion of the diverticulum, extending into perivesical fat and the seminal vesicle.

Findings:

• The diverticular tumor exhibited aggressive histologic features, including high-grade urothelial carcinoma and sarcomatoid differentiation.
• The tumor invaded through the diverticular wall, implicating adjacent structures like the seminal vesicle.
• The paucity of muscle in the diverticulum wall may contribute to the tumor's early invasive behavior.

Implications:

• This case highlights the potential for aggressive malignancies to arise in unusual locations like bladder diverticula.
• The aggressive nature and advanced local invasion suggest a guarded prognosis for this specific tumor type in this site.
• Further research into the pathogenesis and optimal management of carcinosarcomas in bladder diverticula is warranted.
