Summary

Elderly patients with peptic ulcer bleeding showed similar endoscopic treatment outcomes and rebleeding rates compared to younger patients. However, older adults experienced higher overall mortality but lower post-surgery death risk.

Area of Science:

  • Gastroenterology
  • Geriatric Medicine
  • Endoscopic Surgery

Background:

  • Peptic ulcer bleeding (PUB) is a significant clinical challenge, particularly in the elderly.
  • Endoscopic injection therapy is a common treatment for PUB.

Purpose of the Study:

  • To evaluate the efficacy of endoscopic injection therapy in elderly patients with peptic ulcer bleeding.
  • To compare clinical outcomes of peptic ulcer bleeding between elderly (>=70 years) and younger (<70 years) patients.

Main Methods:

  • Prospective observational study of 738 patients with acute peptic ulcer bleeding (January 1995 - December 1998).
  • Patients were divided into two groups: elderly (>=70 years) and young (<70 years).
  • Endoscopic injection therapy with epinephrine and polidocanol was administered for active or recent bleeding ulcers.

Related Experiment Videos

Main Results:

  • Elderly patients had more coexisting diseases, except for liver cirrhosis.
  • Similar rates of endoscopic treatment, rebleeding, and hospitalization duration were observed between age groups.
  • Overall mortality was higher in the elderly (12.7%) compared to the young (8.3%), but post-surgery mortality was lower in the elderly (8.3% vs 57.1%).

Conclusions:

  • Clinical and endoscopic features, along with rebleeding rates, are comparable between elderly and young patients with peptic ulcer bleeding.
  • Elderly patients face higher overall mortality but a reduced risk of death following surgery.
  • Endoscopic injection therapy appears effective in managing peptic ulcer bleeding across different age groups.
