Summary

Buschke-Löwenstein tumours, often linked to Human Papillomavirus (HPV) 11, can present with micro-invasive carcinoma. Complete surgical resection and histological examination are crucial for accurate diagnosis and management of these rare genital lesions.

Area of Science:

  • Dermatology
  • Oncology
  • Virology

Background:

  • Buschke-Löwenstein tumour (BLT) is an uncommon genital neoplasm with controversial classification, ranging from benign giant condyloma to malignant carcinoma.
  • Distinguishing between benign, potentially malignant, and carcinomatous lesions within BLTs is diagnostically challenging.

Observation:

  • A case report details a 34-year-old patient with recurrent condylomatous lesions of the penis and scrotal extension, diagnosed as Human Papillomavirus (HPV) 11-associated Buschke-Löwenstein tumour.
  • Histological examination of the surgical specimen revealed an area of micro-invasive carcinoma within the BLT.
  • Molecular analysis confirmed HPV 11 DNA, and immunohistochemistry indicated basal cells weakly expressing mutant p53.

Findings:

  • The study highlights the presence of HPV 11 in a Buschke-Löwenstein tumour with associated micro-invasive carcinoma.

Related Experiment Videos

  • Histological examination of the entire surgical specimen is essential to detect subtle areas of micro-invasion.
  • Implications:

    • Accurate histological assessment of surgical resections is critical for managing Buschke-Löwenstein tumours, particularly for identifying micro-invasion.
    • While classification remains debated, the findings underscore the potential for malignant transformation and the need for vigilant surveillance post-resection, especially when micro-invasion is detected.
