    Background:

    • Temporal disorientation is a common deficit in patients with brain disease.
    • Conventional neurological examinations may not adequately detect temporal disorientation.

    Purpose of the Study:

    • To evaluate the efficacy of a novel question schedule for assessing temporal orientation in patients with brain disease.
    • To compare the diagnostic accuracy of the new schedule with conventional neurological examination.

    Main Methods:

    • A schedule of questions based on empirically derived norms was administered to nonaphasic and aphasic patients with brain disease.
    • Results were compared to findings from conventional neurological examinations.
    • The frequency of temporal disorientation was analyzed in relation to lesion laterality (unilateral vs. bilateral).

    • The question schedule identified temporal disorientation in 24% of nonaphasic patients, compared to 15% detected by conventional examination.
    • In aphasic patients, the schedule detected disorientation in 33% (5/15), while clinical examination detected only 20% (3/15).
    • Patients with bilateral brain lesions exhibited a higher incidence of temporal disorientation.

    Conclusions:

    • The empirically normed question schedule is a more sensitive tool for detecting temporal disorientation than standard neurological examination.
    • Incorporating this schedule into neurological assessments can improve the identification of temporal orientation deficits.
    • The findings highlight the importance of standardized testing for cognitive functions like temporal orientation.
