Background:

  • Chronic tension-type headache (CTTH) is a prevalent neurological disorder.
  • Selective serotonin reuptake inhibitors (SSRIs) are increasingly explored for headache management.

Purpose of the Study:

  • To evaluate the efficacy of Sertraline, an SSRI, in treating CTTH.
  • To compare Sertraline's effects against a placebo in a randomized controlled trial.

Main Methods:

  • A double-blind, placebo-controlled, randomized trial involving 50 CTTH patients.
  • 10-week study duration: 2-week run-in, 4-week treatment, 4-week follow-up.
  • Assessed headache index, analgesic use, headache frequency, anxiety, and depression (Hamilton Rating Scale).

Main Results:

  • Sertraline group showed a significant reduction in weekly analgesic intake (4.34 to 1.07 tabs/week, p < 0.01).
  • Placebo group had no significant reduction in analgesic intake (3.98 to 2.94 tabs/week, p > 0.05).
  • No significant reduction in headache index or frequency was observed with Sertraline.

Conclusions:

  • Sertraline significantly decreased weekly analgesic consumption in CTTH patients.
  • Sertraline presents a viable alternative to amitriptyline for patients with CTTH who do not respond to or tolerate it.
