Summary

Swallowing problems, or dysphagia, affect 24% of multiple sclerosis (MS) patients and worsen with disability. Manofluoroscopy (MFS) reveals oral phase deficits and, in severe cases, pharyngeal phase abnormalities.

Background:

  • Multiple sclerosis (MS) is a chronic neurological disease affecting millions worldwide.
  • Dysphagia is a common but often underdiagnosed complication of MS.
  • Swallowing difficulties significantly impact quality of life and nutritional status in MS patients.

Purpose of the Study:

  • To determine the prevalence of dysphagia in MS patients and its correlation with disability.
  • To identify key symptoms indicative of dysphagia in MS.
  • To characterize manofluoroscopy (MFS) findings in MS-related dysphagia.

Main Methods:

  • A cohort of 308 MS patients completed a swallowing problems questionnaire.
  • The Johns Hopkins Swallowing Centre questionnaire was used to assess dysphagia severity.

  • Manofluoroscopy (MFS) was performed on 30 dysphagic MS patients; disability was measured using the Expanded Disability Status Scale (EDSS).
  • Main Results:

    • 24% of MS patients reported permanent dysphagia, with prevalence increasing from 24% (EDSS 2-3) to 65% (EDSS 8-9).
    • Coughing/choking during meals (59%) and history of pneumonia (12%) were significant indicators of dysphagia.
    • MFS revealed oral phase deficits in all patients and pharyngeal phase abnormalities in those with EDSS > 7.5.

    Conclusions:

    • Permanent dysphagia is prevalent in MS, particularly in patients with moderate to severe disability.
    • MFS is a valuable tool for assessing MS-related swallowing dysfunction.
    • Pharyngeal phase abnormalities on MFS are specific to advanced MS disability (EDSS > 7.5).
