Background:

  • Globozoospermia is characterized by 100% round-headed sperm, often associated with fertilization challenges.
  • Intracytoplasmic sperm injection (ICSI) success rates can be poor in globozoospermia cases, suggesting underlying sperm nuclear abnormalities.

Observation:

  • A patient with 100% round-headed sperm was analyzed for sperm DNA integrity and chromosome status.
  • Sperm chromatin packaging, DNA fragmentation (apoptosis), and chromosome aneuploidy/diploidy rates were assessed using flow cytometry and fluorescence in-situ hybridization.

Findings:

  • The patient exhibited significantly higher rates of sperm chromatin decondensation (35%) and DNA fragmentation (37%) compared to controls.
  • Despite abnormal chromatin, sperm chromosome aneuploidy (0.16%) and diploidy (0.05%) rates were within normal ranges.

  • This is the first report linking round-headed sperm in globozoospermia with elevated abnormal chromatin structure and DNA strand breaks.
