  • Gross hematuria can be a presenting symptom.

Observation:

  • A 35-year-old male presented with asymptomatic gross hematuria.
  • Pelvic MRI revealed a large, multilocular mass replacing the prostate gland.
  • The tumor was adherent to the displaced urinary bladder.

Findings:

  • Pathological examination confirmed a giant multilocular prostatic cystadenoma.
  • The patient underwent extensive surgical resection.
  • No tumor recurrence was observed at 24 months post-surgery.

Implications:

  • Complete surgical excision may not be universally required for prostatic cystadenoma.
  • Awareness of this rare entity is crucial for accurate diagnosis and treatment planning.
  • Further research into the natural history of prostatic cystadenoma is warranted.
