Summary

Spontaneous chylothorax and chyloptysis, though rare, can stem from thoracic duct reflux into lung lymphangiectasis. A medium chain triglyceride diet proved effective in treating these conditions.

Area of Science:

  • Thoracic medicine
  • Gastroenterology
  • Pulmonology

Background:

  • Spontaneous chylothorax is a rare condition with diverse etiologies, including pleural, pulmonary, or mediastinal origins.
  • Chyloptysis, a rarer clinical issue, is consistently linked to pulmonary causes.

Observation:

  • Two cases are presented: a 63-year-old woman with chylothorax and a 28-year-old man with chyloptysis.
  • Both patients were successfully treated using a medium chain triglyceride diet.

Findings:

  • Lymphangiograms revealed a common origin for both cases: reflux from the thoracic duct into pulmonary lymphangiectasis in the right lower lobe.
  • This suggests that chylous reflux into pulmonary lymphangiectasis may be more common than previously thought.

Implications:

Related Experiment Videos

  • The findings indicate that chylous reflux into pulmonary lymphangiectasis is likely underdiagnosed.
  • Further investigation into this specific etiology for chylothorax and chyloptysis is warranted.
