Summary

A rare aortopulmonary fistula developed in a pseudoaneurysm of the left coronary ostial button after aortic root replacement. This complication requires consideration in patients with new-onset heart failure post-surgery.

Background:

Aortic root replacement for acute type A aortic dissection is a complex procedure.
Post-operative complications can arise, necessitating vigilant patient monitoring.
Pseudoaneurysm formation is a known, albeit rare, risk following aortic surgery.

Observation:

A 66-year-old woman presented with sudden congestive heart failure three years post-aortic root replacement.
The patient had undergone surgery for acute type A aortic dissection.
Physical examination and diagnostic imaging revealed an aortopulmonary fistula within a left coronary ostial button pseudoaneurysm.

Findings:

Preoperative diagnosis was confirmed using aortography, heart catheterization, and spiral computed tomography.

The patient was discharged in stable condition following the corrective surgery.

Implications:

Aortopulmonary fistula is a critical complication to consider in patients experiencing severe congestive heart failure after aortic root replacement.

This case highlights the importance of long-term surveillance for patients with a history of aortic root surgery.
