This study describes a case of exercise-induced anaphylaxis (EIA) triggered by cold exposure, not food. This cold-dependent EIA may represent a distinct clinical entity.

Background:

  • Exercise-induced anaphylaxis (EIA) is a rare condition characterized by allergic reactions during or after physical activity.
  • It is often associated with food triggers, but other factors can also be involved.

Observation:

  • A 16-year-old male experienced recurrent urticaria (wealing) and dyspnea (shortness of breath) after winter exercise.
  • Symptoms were not linked to food ingestion, specifically wheat or soya bean.
  • A provocation test combining exercise with cold stimulation elicited localized skin reactions.

Findings:

  • Cold exposure, rather than food ingestion, was identified as the essential trigger for EIA in this patient.
  • The patient's condition differed from typical cold urticaria and food-dependent EIA.
