Summary

Diagnosing insulinoma can be challenging with normal insulin levels. Increased serum proinsulin levels, detected by proinsulin assays, are crucial for accurate insulinoma diagnosis.

Area of Science:

  • Endocrinology
  • Oncology
  • Clinical Biochemistry

Background:

  • Insulinoma is a rare pancreatic neuroendocrine tumor causing hypoglycemia due to excessive insulin secretion.
  • Typical diagnostic criteria involve detecting elevated serum insulin levels concurrent with hypoglycemia.
  • Atypical presentations can complicate the diagnosis of insulinoma.

Observation:

  • Two patients presented with classic insulinoma symptoms but exhibited normal serum insulin levels.
  • Despite normal insulin, elevated serum proinsulin levels were observed in both cases.
  • Standard insulin assays using monoclonal antibodies may not detect isolated proinsulin elevations.

Findings:

  • Serum proinsulin levels were significantly increased in both patients.

Related Experiment Videos

  • Proinsulin assay proved essential in confirming the diagnosis of insulinoma when insulin levels were normal.
  • Monoclonal antibody-based insulin assays lack sensitivity for isolated high proinsulin.
  • Implications:

    • Proinsulin measurement is a valuable tool for diagnosing atypical insulinoma cases.
    • This finding expands diagnostic capabilities for pancreatic tumors.
    • Enhanced diagnostic strategies are needed for rare endocrine tumors.
