Background:

The Marcus-Gunn phenomenon (MGP) is an unusual synkinesis between the jaw and eyelid.
Inverse MGP (IMGP) is a rare variant, typically acquired, involving eyelid depression with jaw opening.
Congenital ptosis is a drooping of the upper eyelid present from birth.

Observation:

A young male presented with congenital ptosis.
He also exhibited signs consistent with inverse Marcus-Gunn phenomenon.

Findings:

This case represents a rare congenital presentation of inverse Marcus-Gunn phenomenon.
The co-occurrence of congenital ptosis and IMGP in a pediatric patient is highly unusual.

Implications:

This case expands the known spectrum of IMGP presentations.
