Summary

This study details a rare breast cancer case where gallium-67 (GA-67) imaging detected the primary tumor, unlike typical thallium-201 (Tl-201) uptake. Extensive bone marrow metastases were visualized by both imaging agents.

Background:

Primary breast carcinomas typically show thallium-201 (Tl-201) avidity.
Gallium-67 (GA-67) uptake is uncommon in primary breast cancers, limiting its routine use.
Diagnostic imaging plays a crucial role in staging and assessing metastatic disease in breast cancer.

Observation:

A rare case of primary breast carcinoma with bone marrow metastases is presented.
The primary breast lesion demonstrated significant GA-67 avidity.
Conversely, the primary lesion did not accumulate Tl-201.
Extensive bone marrow metastases were identified by both Tl-201 and GA-67 scans.

Findings:

This case highlights an unusual GA-67 avid primary breast carcinoma that was Tl-201 cold.
The patient presented with aggressive metastatic breast adenocarcinoma.
The extensive bone marrow involvement led to pancytopenia and bone marrow failure.

Implications:

This case challenges the conventional understanding of radiotracer uptake in breast cancer.
GA-67 scintigraphy may have a role in specific, atypical breast cancer presentations.
Understanding variable radiotracer uptake is critical for accurate staging and management of metastatic breast cancer.
