Summary

This study found no significant differences in comorbid personality disorders between melancholic and nonmelancholic major depressive disorder (MDD). These findings challenge the clinical utility of assessing premorbid personality features in depressed patients.

Area of Science:

  • Psychiatry
  • Clinical Psychology

Background:

  • Major Depressive Disorder (MDD) classification has evolved, with previous criteria considering the absence of personality disturbance.
  • The melancholic subtype of depression has been a focus of diagnostic research.

Purpose of the Study:

  • To investigate whether rates of comorbid personality disorders differ between melancholic and nonmelancholic subtypes of MDD.
  • To evaluate the clinical relevance of assessing premorbid personality features in depressed individuals.

Main Methods:

  • A cohort of 260 outpatients diagnosed with Major Depressive Disorder (MDD) were evaluated.
  • Personality disorders were assessed using the Structured Clinical Interview for DSM-III-R-Personality Disorders (SCID-II).
  • The melancholic subtype was identified using a DSM-IV checklist, and depression severity was measured with the Hamilton Rating Scale for Depression (HAM-D-17).

Related Experiment Videos

Main Results:

  • No significant differences in age, gender, or overall rates of personality disorder diagnoses were found between melancholic and nonmelancholic MDD groups.
  • Rates of individual personality disorder clusters were comparable across both depression subtypes.
  • The study included 102 patients with melancholic depression and 158 without.

Conclusions:

  • Comorbid personality disorder rates are similar in melancholic and nonmelancholic major depressive disorder.
  • Findings question the diagnostic utility of excluding premorbid personality disturbance for melancholic depression.
  • The assessment of premorbid personality features in acutely depressed patients may not be clinically useful.
