Background:

  • Transitional cell carcinoma (TCC) is a recognized histologic pattern of primary fallopian tube carcinoma.
  • Extratubal TCC associated with the fallopian tube is rare and often unrecognized.

Observation:

  • A 56-year-old postmenopausal woman presented with a left adnexal high-grade TCC.
  • The tumor was attached to the left fallopian tube's serosal surface via a stalk, separate from the uterus and ovary.
  • Histology and immunohistochemistry (pankeratin, calretinin, ER, PR, CK7 positive; CK20 negative) suggested Mullerian derivation.

Findings:

  • This case represents the fourth reported instance of primary paratubal TCC.
  • The tumor originated extraluminally, with no involvement of the fallopian tube lumen.
