Summary

Patients with corticobasal degeneration may exhibit abnormal manual approach behavior, known as closing-in, during hand gesture copying. This study quantified this behavior, finding it more prevalent with the left hand, and suggested frontal lobe dysfunction may be involved.

Background:

  • Closing-in, a phenomenon where individuals draw near or into a target, is recognized in figure copying and hand gesture imitation.
  • Corticobasal degeneration (CBD) is a progressive neurodegenerative disorder affecting movement and cognition.

Observation:

  • A patient with severe ideomotor apraxia due to probable CBD displayed closing-in behavior when copying hand gestures.
  • Positron emission tomography revealed hypometabolism in the bilateral parietotemporal and right frontal lobes of the patient.

Findings:

  • The patient exhibited closing-in behavior in 53.8% of trials, involving approaching, touching, or grasping the examiner's hand.
  • Closing-in was more frequent and severe when the patient used their left hand compared to their right hand.

  • No significant differences in closing-in were observed between lateral and across conditions or between simple and complex gestures.

    • Implications:

    • Corticobasal degeneration may be linked to aberrant manual approach behaviors.
    • Findings do not support a working memory hypothesis for closing-in in this patient.
    • Frontal lobe dysfunction in CBD might impair voluntary control over the primitive tendency to move towards a visual target.
