Electrocardiographic abnormalities in leptospirosis.

S Ramachandran

The Journal of Tropical Medicine and Hygiene
    |September 1, 1975

    Summary

Sub-clinical cardiac involvement is common in leptospirosis, affecting nearly half of patients. Electrocardiogram changes indicate myocardial issues, necessitating monitoring, especially in acute kidney injury cases.

    Area of Science:

    • Cardiology
    • Infectious Diseases
    • Internal Medicine

    Background:

    • Leptospirosis is a zoonotic infection with potential systemic complications.
    • Cardiac involvement in leptospirosis is not well-characterized.
    • Electrocardiographic (ECG) findings can indicate myocardial dysfunction.

    Purpose of the Study:

    • To investigate the prevalence and nature of electrocardiographic abnormalities in patients with leptospirosis.
    • To assess the significance of these findings for sub-clinical cardiac involvement.

    Main Methods:

    • Retrospective analysis of electrocardiograms (ECG) from patients diagnosed with leptospirosis.
    • Evaluation of ECG parameters including rhythm, P-QRS-T complexes.
    • Correlation of ECG findings with clinical presentation and renal status.

    Main Results:

    • Electrocardiographic abnormalities were observed in 46% of leptospirosis cases.
    • Abnormalities included cardiac rhythm disorders and alterations in P-QRS-T complexes.
    • These changes suggest sub-clinical myocardial involvement.

    Conclusions:

    • Leptospirosis frequently causes sub-clinical cardiac involvement, detectable by ECG.
    • Serial ECG monitoring is recommended for patients with acute renal failure (oliguric) and other presentations of leptospirosis to detect cardiac changes.
