Autologous purified pearl fat (APPF) combined with basic fibroblast growth factor (bFGF) effectively prevented peridural adhesions after laminectomy in rabbits. Other methods showed limited or no success in preventing these adhesions.

Area of Science:

  • Neurosurgery
  • Regenerative Medicine
  • Biomaterials Science

Background:

  • Peridural adhesions are a common complication following laminectomy.
  • Existing methods for preventing these adhesions have limitations.

Purpose of the Study:

  • To evaluate the efficacy of autologous purified pearl fat (APPF) combined with basic fibroblast growth factor (bFGF) in preventing peridural adhesions postlaminectomy.
  • To compare APPF with and without insulin, APPF alone, sodium hyaluronate (SHA), and an uncovered control group.

Main Methods:

  • Laminectomy was performed on rabbits, followed by application of different treatments to the exposed dura.
  • Treatments included APPF with bFGF, APPF with insulin, APPF alone, 2% SHA, and no treatment.
  • Histological and computed imaging analyses were conducted at various postoperative time points.

Related Experiment Videos

Main Results:

  • Complete prevention of peridural adhesions was observed in the group treated with APPF and bFGF.
  • Severe adhesions formed in groups treated with APPF (alone or with insulin) and SHA, as well as the control group.
  • bFGF promoted revascularization and survival of APPF, while insulin showed no benefit and SHA's effect diminished over time.

Conclusions:

  • APPF combined with bFGF is a promising new approach for preventing postlaminectomy peridural adhesions.
  • This combination enhances fat survival and adhesion prevention.
  • Current alternatives like SHA are less effective in the long term.
